Employer-Provided Health Insurance Offer & Coverage (IRS Form 1095-C)

INFORMATION ABOUT IRS FORM 1095

Dear UCSC employee,

In 2010, Congress passed the Affordable Care Act (ACA) which requires every U.S. Citizen and resident alien to have health insurance coverage. Per IRS requirements, the University of California will annually provide proof of available coverage to all full time employees and members of UC’s self-insured medical plans.

On or before March 31, you will receive Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) by U.S. Mail from UnifyHR, the agency UC has contracted with to provide this information. The 1095-C describes what coverage was made available to you, whether you chose to participate or not. The form will be mailed to the home address UC currently has on file for you. (You can review your address information through At Your Service Online.)

If you are enrolled in a fully insured plan (all plans except Core and UC Care), you will also receive Form 1095-B (Health Coverage) in the mail directly from your insurance carrier. Form 1095-B provides details about your actual insurance coverage, including who in your family was covered, and for what periods during the year. It will be mailed to the address you have on file with your insurance carrier. Depending on the type of coverage you have, you may receive one or more of Forms 1095-B or 1095-C.

You do not need to submit the 1095 forms with your tax return, or take any other action, but you should check the forms for accuracy and retain them in the event they are needed to provide proof of coverage.
